How it works

You'll need to sign the CMS Health Tech Ecosystem Pledge before getting started. Learn more about the Pledge

Implement identity verification standards with either ID.me or CLEAR.

Connect to CMS-aligned networks and implement security and interoperability standards.

Complete a third-party vetting review through the DiMe Seal or CARIN Alliance-CFA program

Submit your app for review for being listed in the Medicare App Library.

Replace clipboard forms with FHIR-based data exchange. Accept verified patient identity and health history at the point of care via QR code, Smart Health Card, or Smart Health links, returning a structured visit summary.

Conversational AI assistants

Build AI-powered tools that access clinical records through a CMS Aligned Network to deliver personalized guidance. Responses must be clearly labeled as AI-generated and distinguish educational content from clinical advice.

Diabetes & weight management

Connect to CMS Aligned Networks to power tailored coaching, risk alerts, and chronic disease support. Apps must address both prevention and active management, including specific resources for pre-diabetic patients.

Get listed in the Medicare App Library

Signing the Health Tech Ecosystem pledge is your first step toward being listed in the Medicare App Library. Your app must meet CMS pledge requirements across these areas: security and privacy, interoperability, and independent accreditation.

Once you've fulfilled your pledge, submit your app to CMS for review. If all criteria are met, your app will be included in the Medicare App Library.

ACCESS Model participants are exempt from CMS Review and Trial Access requirements and receive a special designation in the Medicare App Library.

Identity verification

Verify patient identity using CMS-approved services that meet IAL2 and AAL2 standards. Partner with ID.me or CLEAR to implement secure identity verification.

Connected circle nodes

Interoperability

Connect to the CMS health data ecosystem using open standards. Your app must support FHIR R4, SMART on FHIR authorization, and integrate with a CMS Aligned Network.

Accreditation

Complete independent evaluation through a CMS-recognized vetting partner. Choose either the DiMe Seal or CARIN Alliance Code of Conduct with DirectTrust Accreditation.
