The 4-digit Type of Bill code appears as follows:
- The first digit is always a placeholder zero. It only appears on paper forms and isn't submitted on electronic claims.
- The second digit identifies the type of facility; for example, a hospital.
- The third digit classifies the type of care; for example, outpatient care versus inpatient care.
- The fourth digit can be a letter or number that indicates the sequence or frequency of the bill within a particular episode of care; for example, interim.
- 0123 – Hospital inpatient (interim continuing claim)
- 0131 – Hospital outpatient (admit through discharge claim)
- 0811 – Hospice (nonhospital-based, admission or initial bill)
- 0711 – Clinic: Rural Health Clinic (RHC – admit through discharge claim)
