Noridian's Response to Provider Recommendations for Botulinum Toxin Types A and B (for comment period ending 08/08/2016)

1 A botulinum toxin manufacturer recommended that we remind physicians to use each product based on the FDA package insert. Although some of the botulinum products may be used by physicians in an off-label manner, we do believe physicians should use labeled indications as much as possible. Therefore we will amend language to recommend that: “Providers should consult the package insert of each neurotoxin to identify the FDA approved indications for each product.”
2 A botulinum toxin manufacturer suggest we not discuss FDA approval of individual products as some projects have joint approval. Noridian removed most specificities of individual products—recommending that physicians consult the FDA product brochure or manufacturers’ websites for specific FDA usage.
3 Commenter asked that we remove the exclusion for“secondary strabismus caused by prior surgical over-recession of the antagonistic muscle”, as a trial of botulinum toxin may be preferable to re-operation. Noridian will accomplish that.
4 Commenter requested the addition of other diseases of the larynx and anal fissure, unspecified. Noridian feels there needs to be more specific codes and will not change that section.
