One comment is that I think that the term actigraphy should be clearly defined in the NCD - and with something more specific than just a "measure of movement". That seems just way too vague. It seems that there should be some discussion on what part(s) of the body, how measured, how much movement, how is presence or absence of movement used to establish dx of OSA, etc.

Nevertheless, as a Sleep Specialist, my experience with Type III or IV devices in unattended studies has demonstrated several problems. Especially in patients with mild sleep apnea or snoring alone, these devices OVER diagnosis the problem. Patients then end up with CPAP machines prescribed for them who do not need this therapy. These patients may end up being retested in a

1. This draft NCD does not follow the American Academy of Sleep Medicine recommendations regarding sleep testing. The draft NCD is more liberal in allowing any physician (not just a Board certified Sleep specialist) to interpret sleep tests. There may be other differences as well. With Type II-IV testing, there may be spurius results and overtreatment by up to 15% (a serious cost-benefit concern, if not a concern for those patients mis-diagnosed) as well as undertreatment.
