Computed tomographic angiography (CTA) is a noninvasive method, using intravenous contrast, to visualize the coronary arteries (or other vessels) using high resolution, high speed computed tomography (CT). CTA, which includes helical (spiral) CT and multi-slice CT angiography, has 2 potential uses: 1) a substitute for invasive coronary angiography and 2) evaluation of chest pain in the emergency room.

Proponents have claimed that the use of cardiac CTA will lead to better health outcomes and a reduction in cardiac catheterization, the current standard for diagnosis of coronary artery disease.

CMS is concerned that despite the lack of clinical evidence to demonstrate improved patient health outcomes with CTA, the procedure has been rapidly adopted by the clinical community.

This NCA will evaluate the available evidence for CTA when used to diagnose coronary artery disease and determine if a national coverage determination is warranted. We will also consider the potential of Coverage with Evidence Development as an appropriate NCD decision.

CMS initiates this national coverage analysis for the use of computer tomography for angiographic study. The public has 30 days to submit comments on this topic. CMS considers all public comments, and is particularly interested in clinical studies and other scientific information relevant to the technology under evaluation. We are especially interested in recommendations as to the types of studies needed if the evidence is determined to be premature for coverage.
