The CMS Innovation Center established the Value-Based Care Spotlight site to help people better understand value-based care and its role in improving health care and the patient experience.

For example, a home health companion provided through a value-based care pilot program helped a Maryland artist get back on his feet after a major knee injury resulted in loss of mobility and isolation. Meet a retired chef, who after being diagnosed with diabetes, wanted to lower his A1C without taking medication: go.cms.gov/vbc-chef-story. His doctor worked with him to develop a health plan to address his diabetes while respecting his desire to not take medication. As a result, the patient was able to make dietary changes and participate in more regular exercise to meet his health goal. He even started a walking group to inspire others to move more. #WhatDoYouValue
