Learn how through value-based care a retired chef partnered with his doctor to create a plan to address his diabetes that best met his health goal of not taking medicine: go.cms.gov/vbc-chef-story #WhatDoYouValue LinkedIn or Facebook #ValueBasedCare is designed to improve your health care experience, which includes providers who take time to get to know you and your health goals. Meet a retired chef, who after being diagnosed with diabetes, wanted to lower his A1C without taking medication: go.cms.gov/vbc-chef-story. His doctor worked with him to develop a health plan to address his diabetes while respecting his desire to not take medication. As a result, the patient was able to make dietary changes and participate in more regular exercise to meet his health goal.
