Due to popular demand, the Comprehensive Primary Care Initiative Plus (CPC+) team hosted multiple repeats of the webinar that was originally held on Thursday, April 14, 2016 and Tuesday, April 19. The repeat webinars were held on Tuesday, May 3 and Wednesday, May 11, 2016. During these webinars Model team members provided an overview of the model.
