For cost reporting periods beginning on or after October 1, 2014, Freestanding FQHCs are required to file using the Freestanding FQHC Cost Report (Form CMS 224 14), not the Independent (RHC)/Freestanding FQHC Cost Report (Form CMS-222-92).
