Evaluation of End Stage Renal Disease (ESRD) Disease Management (DM)

This Task Order is for an independent evaluation of the ESRD-DM Demonstration (DMD) that will examine case-mix, patient and provider satisfaction, outcomes, quality of care, costs, and payments. The Request for Proposals for providers to participate in the DMD was published in the Federal Register on June 4, 2003. The DMD will enroll Medicare beneficiaries with ESRD into fully capitated ESRD disease management organizations. When the DM sites are selected, the evaluation team will work with them to design and implement data collection instruments and mechanisms. The evaluation contractor will also work with the DM sites to collect and analyze data to measure clinical, quality of life, and economic outcomes.
