Description
The purpose of this project is to independently evaluate chronic care improvement programs implemented under the developmental phase (Phase I) of the Voluntary Chronic Care Improvement Under Traditional Fee-for-Service Medicare initiative as authorized by Section 721 of the Medicare Prescription Drug, Improvement, and Modernization Act of 2003 (Public Law 108-173). These pilot programs have been implemented under the name 'Medicare Health Support.' Eight organizations implemented care management programs in different geographic regions between 2005 and 2008. In each region, approximately 30,000 Medicare beneficiaries with heart failure or diabetes were identified as eligible; 20,000 were offered the intervention, and the remaining 10,000 served as a comparison population.
