Description
This task order supports the Centers for Medicare and Medicaid Services (CMS) in implementing approximately six regional programs to provide care management services to high cost Medicare fee-for-service beneficiaries under the Care Management for High-Cost Medicare Beneficiaries (CMHCB) demonstration. The assumption is that 8,000 beneficiaries will be placed in an intervention group and 8,000 in a control group for each of the 6 programs, yielding 80,000 to 120,000 beneficiaries for ongoing analysis.
