SAN Power Packs

A key activity for the SANs to spread practice transformation successes was the development of an approach for defining service delivery challenges and presenting solutions that have been successfully implemented by practices. The resulting product, referred to as a Power Pack, identifies service delivery bundles that offer high value for specific situations, including for population groups and organizational situations that are well-defined and offer opportunities for achieving value-based performance.

Power Packs are breakthrough examples of practices that addressed challenges to:

  • Improve health outcome;
  • Avoid unnecessary use of hospitals and emergency departments;
  • Avoid unnecessary tests and procedures; and
  • Reduce the total cost of care.

In addition, Power Packs:

  • Support short- and long-term financial sustainability;
  • Improve patient engagement and satisfaction; and
  • Improve provider joy in work, including employee satisfaction and workforce retention.

The SAN Power Packs are examples of innovative transformational accomplishments that address specific service challenges to delivering value-based care organized in a core set of 6 topic areas for practice improvement.

Sustainable Business Operations

Practices are missing strategic financial management infrastructure needed to thrive and transition to value-based payment models. Power Packs in this topic describe how practices add leadership and develop the infrastructure to become financially viable and sustainable.

 

Person and Family Engagement

Patients face difficulties in fully engaging in their care, resulting in missed or canceled appointments, negatively influencing health outcomes and practice efficiency and profitability. Power Packs in this topic describe how practices address social determinants of health to decrease rates of no-shows, improve financial performance, and result in better health outcomes.

 

Joy in Work and Satisfaction

Clinician burnout results in high levels of stress and dissatisfaction which negatively impacts the patient experience of care. Power Packs in this topic describe how practices address workforce needs that result in improved engagement and job performance.

 

Understanding Data

Practices are having trouble analyzing and understanding their existing data sources, leading to difficulties in adopting alternative payment models. Power Packs in this topic describe how practices analyze their data to find inefficiencies and adopt changes that improve health outcomes.

 

Evidence-Based Care

Many practices receive conflicting information on clinical best-practices, preventing them from implementing evidence-based quality improvements. Power Packs in this topic describe how practices use clinical guidelines and recognize sources of evidence-based approaches to care.

 

Care-Coordination

Patients often need a set of coordinated services from multiple providers. The referral and transition process is difficult to navigate, resulting in poor outcomes and inappropriate or unnecessary utilization of services. Power Packs in this topic describe how practices coordinate care, such as behavioral health and primary care, in ways that lead to improved engagement, efficient patient flow, and better health outcomes.
