Professional Associations & Public-Private Partnerships in Transforming Clinical Practice Initiative - Support & Alignment Networks (SANs)

The ten professional associations and organizations that participated in TCPI as SANs supported clinicians in value-based care delivery and prepared them for participation in Alternative Payment Models (APMs) by:

  • Enabling transformation through their existing and emerging assets, programs, and communications channels;

 

  • Spreading best practices to their 350K+ primary and specialty care clinician membership and professional sectors; and

 

  • Ensuring sustainability and continued diffusion of learning within the health care community.

2019 SAN Program Focus Areas

In addition to supporting clinicians and practices enrolled in TCPI, these associations worked on several activities to spread best practices and ensure sustainability and diffusion of learning.

 

Exemplary Practice Brief (PDF) 

Each SAN’s Exemplary Practice Framework articulated a unique vision of care delivery for their professional sector and spheres of influence. This brief includes an overview of the frameworks, impact on the work of the SANs, and plans to leverage these Exemplary Practice Frameworks to support transformation toward value-based care.

 

Professional Association Reach and Dissemination (PDF) 

This document provides an overview of the approximate membership and wider influence of each SAN organization, as well as their outreach activities and communications channels to distribute information about the TCPI model and prepare their professional sectors to transition to value-based care. 

2019 Annals of Family Medicine Special Issue

“Lessons from Practice Transformation” includes early findings from multiple examples of health extension and practice transformation support, including several articles from members of the TCPI community. SAN Power Packs 

The Power Packs are examples of innovative transformational accomplishments at the practice level that address specific, discrete hurdles on the pathway to preparing a clinical practice to adopt an alternative payment model (APM).  The Power Packs address challenges in six core topics for practice improvement:

  • Understanding Data
  • Evidence-Based Care
  • Care-Coordination

2019 SAN Summit

The SANs worked with CMS to engage additional medical associations and support a wider audience of health care providers in delivering value-based care. The culmination of these efforts was the 2019 SAN Summit (Agenda (PDF), titled “Professional Associations as Partners in Delivering Value-based Care,” held with the following additional 18 medical associations in attendance:

The Summit showcased strategies employed by the SANs to support clinicians and practices in delivering value-based care through workforce skills development and innovative quality improvement programs. The Summit aimed to establish a critical mass of association commitments to preparing their clinician members for participation in APMs and other value-based payment arrangements.

SANs and invited associations shared specific commitments to support their members in transitioning to value-based care to ensure a powerful legacy for the next year after the close of the TCPI model, which included SANs being a resource to the invited associations and continued engagement of professional organizations with CMS on policymaking and program development.
